With the identification of genes and molecular imaging markers that confer prognostic or therapy-related information in prostate cancer, the importance of appropriate and equitable use of this information has become critical. Several groups recommend the use of these molecular tools in patients with high- or very high-risk localized disease and recommend germline and somatic genetic testing in patients with metastatic prostate cancer.
Molecular imaging has also become central to appropriate diagnosis and disease monitoring. In this article, we outline the rationale supporting these recommendations and discuss the potential pitfalls encountered by clinicians in practice, including the equitable application of these tools so that all patients will benefit.
